Applicants will use this template to apply for a Certificate of Authority. The application should be used by institutions working toward Coordinating Board-recognized institutional accreditation.

The head of the department in which the eligible firefighter or law enforcement officer was employed (or volunteered) at the time the incident occurred must file a certificate with the Texas Higher Education Coordinating Board using this template (see Texas Education Code, Section 54.351).

This form is used to collect supporting documents when applicants are applying for the Children of Disabled or Deceased Firefighters and Law Enforcement Officers exemption.

Proposal form to establish a Multi-Institution Teaching Center, Single Institution Center, or University System Center.

This report collects data for the FY 2022 -FY 2023 biennium which includes the 2021-22 (FY 2022) and 2022-23 (FY 2023) award years on students who were admitted into a Teacher Education Program, Bilingual Education, or other comparable program for teacher education in English as a Second Language, or Spanish; and those who were awarded through the Bilingual Education Program.

Texas public institutions of higher education must complete this form to propose: (1) Bachelor’s or Master’s Degree programs in engineering; (2) Bachelor’s or Master’s degree programs that have an estimated cost of more than $2 million in the first five years of operation; and/or (3) Bachelor’s or Master’s degree programs that do not meet the certification requirements set forth in Texas Administrative Code (TAC), Title 19, Chapter 5, Subchapter C, Section 5.44 (a) (3).
